Oregon CUSD #220 Orientation System

ZOKA Media Group LLC (“Producer,” “we,” “our,” or “us”) is the exclusive owner and operator of this orientation system, custom-developed for Oregon CUSD #220 (“Client”). By accessing or using this platform, you and your guardian agree to the terms and conditions outlined below.

1. Purpose of the System

This orientation platform is designed to:

  • Educate students and guardians on Oregon CUSD #220 athletic policies and expectations
  • Facilitate digital acknowledgments of compliance
  • Centralize student-athlete orientation completion records
  • Ensure that orientation is completed in a single uninterrupted session
2. Information We Collect

We collect only the information necessary to verify and record completion:

  • Student First and Last Name
  • Student Email
  • Student ID Number
  • Parent/Guardian First and Last Name
  • Parent/Guardian Email and Phone
  • Digital Signatures and Timestamps
  • IP address and general geolocation (city, state, country flag)

Additional data collected may include browser user agent strings and technical metadata for internal security or troubleshooting.

3. How We Use & Store Information

All information collected is used solely for Oregon CUSD #220 administrative purposes. Data is securely stored on servers leased and maintained by ZOKA Media Group. Data hosting is provided by a third-party provider (NixiHost). Form submission data may also be accessed through administrative tools used by the district and may be archived using Google Workspace or Google Sheets for internal records.

Producer does not guarantee absolute data security and shall not be held liable for any accidental exposure, breach, or unauthorized access.

Data is retained for the duration of the district’s use of the system. If license access lapses, data will be preserved for a short grace period before permanent deletion. Requests for data removal must be submitted by email to [email protected] and confirmed by phone at (312) 636-6156, including the specific records to be deleted.

4. Cookies & Web Behavior

When using the site, we may collect cookies to improve functionality:

  • Login and session preferences
  • Browser acceptance and repeat visitor convenience
  • Form field persistence within a single session only

If you leave a comment, you may opt-in to saving your name, email, and website in cookies for convenience. These cookies may last for up to one year. Login and display preference cookies may also be used for users with backend access.

5. Media Uploads

If users are able to upload media, they should ensure that location data (EXIF GPS) is not embedded, as other users may download and extract such information.

6. Embedded Content

Pages on this site may include embedded content (e.g., videos, images, articles). Embedded content from other websites behaves as if the visitor accessed the third-party site directly and may include their own tracking or data collection.

7. Your Rights Over Your Data

If you have submitted personal information, you may:

  • Request a copy of the data we hold about you
  • Request removal of specific data, subject to district approval
  • Request correction of inaccurate information

This does not include any data we are obligated to retain for legal or administrative purposes.

8. Who We Share Data With

If password resets or system-generated communication is triggered, IP address and associated user metadata may be included in those communications. Data is not sold or shared with advertisers.

9. Use by Minors

This system is intended for high school students and their guardians. Users under the age of 18 must complete the orientation jointly with a parent or guardian. This site is not intended for independent use by children under 13.

10. Legal Responsibility & Limitation of Liability

Producer makes no warranty regarding uninterrupted service or data security and shall not be held liable for damages resulting from platform use, technical issues, or unauthorized access.

Client assumes responsibility for ensuring compliance with all applicable laws including FERPA, SOPPA, and COPPA. All user activity on this platform is logged for verification and audit purposes. Unauthorized use or tampering may result in disciplinary or legal consequences.

11. Accessibility Statement

We are committed to making this orientation system as accessible as reasonably possible for all users. The system was built using tools that support accessibility best practices, including Elementor, Gravity Forms, OnlineADA.com tools, and the MaxAccess accessibility plugin for WordPress.

Producer follows the published accessibility guidance of these platforms and incorporates features such as:

  • Proper HTML5 markup and semantic structure
  • Clear navigation and tab order
  • Sufficient color contrast
  • Alt text support for images and visual media
  • Screen reader compatibility where possible
  • Automated accessibility enhancements through MaxAccess

Despite our efforts, full WCAG 2.1 AA compliance is not guaranteed. Clients are responsible for conducting final audits or legal reviews related to accessibility requirements and certifying compliance with applicable laws.
